Reaching Home partners with community agencies in Prince Albert to facilitate intakes and housing assessments. Housing Support workers workers attend locations around the city to complete intakes with people experiencing homelessness or at risk of experiencing homelessness, where they will then be matched by a Coordinated Access with a housing support worker to receive support to obtain housing.
